How to Become a GAP Guardian

Duties of a GAP Guardian

  • Visit the Ward twice a month to check on his/her health and well-being
  • Advocate for the Ward’s needs
  • Make medical and other personal decisions
  • Participate in care planning
  • Attend CASS quarterly educational trainings
  • Document contacts with the Ward on quarterly reports
  • Complete biennial reports for the Probate Court

To Become a GAP Guardian
Laurie Luken, the GAP program manager will guide you through the process. Contact her at (513) 559-4488 or by email at lluken@cassdelivers.org. You will attend a two-hour orientation program and complete the application form. At this point, the process begins to match Guardians and Wards. Once matched, you will sign the guardian application documents with an attorney and attend a Probate Court hearing for appointment as a Guardian.
